Plan Duration:

The plan is valid for one year from the enrollment date and automatically renews each year unless canceled in writing before renewal or if the annual fee is not paid. Associated Dentists may discontinue the plan at the end of any term with written notice

Plan Payment Options:

Yearly payments may be made by cash, check, or credit card. Payment for any additional dental services not covered under preventive care is due at the time of service to receive the 15% plan discount. Interest-free payment plans for additional services are available upon request with approved CareCredit financing.

Plan Participation Guidelines:

The Associated Dentists Care Plan is a dental membership program—not insurance—and is valid only at Associated Dentists. Memberships are non-transferable and available exclusively to patients without existing dental insurance. Members are responsible for scheduling and keeping appointments. No refunds are issued for dues paid, including missed or unscheduled visits. Please provide at least 48 hours’ notice for appointment changes. Fees for services outside the included preventive care may change at any time.
